Enable defining the input field name in the form editor

The headers/labels for forms need to be more concise for downstream users to integrate or to link in the calculated fields. They sometimes have "Untitled X" and it's not clear where to edit it. You also have to submit a form first before you can change the header of the spreadsheet to not use these labels. I think text and input mapping link should be a little clearer to break/connect/configure.
